How about a day at a spa for the 3 of you. I think your time might be something she would really love.
If she isn't able to clean her house as good as she once did, how about maid service.
Does she have an old friend she hasn't seen for a while? send for her or you take her there.

A giant (useful) basket filled with heaps of small things each individually wrapped...the kinds of things you don't buy for yourself...maybe scent, special soaps, gourmet tea or coffee.

A digital photo frame? Load it with family photos from all eras, and let it flick through, we did this for my dad this Xmas adn he loves it.

I bought a porcelain doll and made a special dress and hat. I gave it to Hazel, she was about 90 y.o. and she cried. "That is the first doll I ever got in my life. You could have heard a pin drop at that party. That doll had a special place on her bed after that.

i think the idea of a monthly gift (i.e. fruit, flowers, candy, books, etc.) is great. a continuing present all year long. i'm sure that at her age she doesn't really need anything more to dust, clean or feed. i once got chocolate of the month and couldn't wait for the next month.
