Funny hospital charts
 
In some cases it’s easy enough to figure out what they were trying to communicate. In other cases, all you can do is scratch your head and laugh!


Patient has chest pain if she lies on her left side for over a year.

The patient has been depressed since she began seeing me in 1993.

Patient had waffles for breakfast and anorexia for lunch.

Rectal examination revealed a normal size thyroid.

I saw your patient today, who is still under our car for physical therapy.

Both breasts are equal and reactive to light and accommodation.

Patient has left white blood cells at another hospital.

Examination of genitalia reveals that he is circus sized.

The lab test indicated abnormal lover function.

Skin: somewhat pale but present.

Actually, if these are real, it is kind of scary. My brother who had a glass eye was in the hospital for some tests years ago. The head nurse came into his room and kept asking him if he felt okay. She left and another person came into his room to put a tongue depressor near his bed which he thought was very strange. Finally, the head nurse revealed that the pupils in his eyes were not equal and that she was worried about a seizure, etc. Now the glass eye information should have been on his chart. By the way the doctor reacted when my brother told him about this incident, I think the information was there. The doctor and nurse had a very serious discussion behind closed doors.

It is amazing what gets into charts. Doctors mumble, their handwriting is miserable, and some talk so fast when doing dictation, you may as well quit! I once went to see a new doc on staff and told him that I had to do his work but if he persisted in talking 90 miles an hour, I would not be responsible for what showed up in his records. I had to to this about 4 times, but he finally got the message!
