These are 4" square Kona muslin (the good muslin!) Cathedral Windows frames. They start out with a 10.5" square.
What is a fair price to ask for them per piece, and in quantities of 16 or more?

I have no clue but i'm going to watch this because I might be interested.
I would take cost of supplies (10-12 per yard?) and add in a reasonable amount for the time it takes to make one.
However, many times, you find that by the time you do that, they might be overpriced for the marketplace. I would probably be willing to pay between $1-2 each. Whether that is fair or not, if it were more than that, I would probably just choose to make them myself.
I think $1 to $2 each is a little expensive. I do think you show very good workmanship. I have made many of these, and they don't take much time to make, but it does take quite a few of them to make a quilt. This is my take along project, and I've been working on it for about 5 years. Haven't worked on it for awhile though.

when I bought kaleidosocpe 72 peices it was 21.95. when I wanted them sewn it was 1.25 a peice just another thought
