Unhappy Best wording for quilt label.

A friend ask me to piece together a quilt with blocks made by her grandmother. What is the best way to word the label without offending anymore? Should I place my name on the label or should I only have the the lady who made the blocks? Any suggestions would be helpful. Also I have not gotten any e-mails from the board. I used to received one everyday, but have not gotten any e-mails in months. Any idea how I can fix this??
    Thanks all!

I have used wording like "Blocks made (pieced, sewn, whatever) by XXX, Quilt assembled and finished by YYY".

I recently finished a quilt for a friend who's grandma had made the blocks. We decided to call it
"From Granny's Hands" and then added:
Blocks made by ........................(grandma's name) est. date ............
    Completed for .....................(friend's name)
Sewn and quilted by ............... (my name)
    completion date.......

I also think t's suggestion is very appropriate and provides a lot of historical information for future generations.
