You'll know it when you see it cause it looks just like eyelashes. It happens on curves and can be caused either by tension being off or hand speed/machine speed out of coordination.
Eyelashes on the bottom for example. The top thread will appear in loops on the bottom of the work and as you go around the curve the bobbin thread will sit above the work and pull heavier on the apex of the curve forming longer "lashes". Do a search on eyelashes here and you'll probably see some photo's. difficult to explain what it looks like.

I found when that happened to me it was my walking foot although I was swearing it wasnt. I purchased another walking foot and it was resolved. It occured whether I slowed down or quilted at a regular speed.
