Rectangle Casserole Carrier pattern/simple instructions?

Hi-
Does anyone know of a simple pattern, or even a basic way to create one of these things? I want it a rectangular shape. I've searched the net, and I haven't found a pattern I like yet..I don't want any dowels for handles. Any ideas??

You can take two quilted placemats, sew both long sides and one short end and turn. Place four buttonholes (two in each unsewn end. You place a wooden spoon through the buttonholes and that closes the carrier. I saw it on the internet but haven't made one yet. They did embroidery on the top placemat.
