How about something on the lines of

Made from mom's wedding dress

Aside from "Made by Grandma", I'd ask the Mom what she wants it to say. If she doesn't know, Made by Grandma works just fine.

I like Snooze2978's suggestion. The only thing I would do a little different is
(Mom's name and Dad's name) joined (date of marriage)
We added (baby's name and date of birth)
This Makes Our Family Whole (or something like that)

Then underneath

this was lovingly made for "baby's name" on (date) by "Mimi" with pieces of
the dress worn on the date this all began

Why not make two labels: 1) with the words from the parents wedding announcement and 2) with the words from the birth announcement. For my daughter's surprise gift of DWR wedding quilt, my BFF used her fancy embroidery machine to make the label that contained the words from my DD and SIL wedding invitation. When my DD saw the quilt, she cried, then when she read the label, she cried again! Made it ALL worth it!!

Beautiful quilt. I came up with this for the label - This quilt is made with pieces of Mommy and Daddy's first special day, Given with the love sewn together for their second special day. Happy Birthday (baby's name).

Wow! A lot of good suggestions--now to choose! I like AZ Jane's suggestion to ask the Mom. I think that's what I'll do and I'll present her with a lot of suggestions--from all of you I'm currently pinning together the quilt sandwich. I do some things the old-fashioned way.
