Thank you! The pattern is Simplicity 4993. Apparently it is out of print - I searched online for hours and couldn't find one to purchase. Then hubby and I went for a drive to the mountains (Canmore, Alberta, Canada) and in the wonderful quilt store there I found several of these patterns!
