Standard treadle sewing machine
#2
It looks to me as it is a Standard Rotary Slim. It is missing a handle on one of the drawers and a slide plate for the machine. Are the missing pieces in the drawers? Does it have a bobbin case? I can't tell, but it looks like it is also missing a piece for the left front.

As mentioned in one of the above links that Standard didn't keep records about serial numbers. I believe in one of the about links they thought it may be early 1900s. It looks like it has been well taken care of, or nicely restored.
You can also search Quilting Board in the search box in the upper right corner of this page (or any other page of Quilting Board).

Yes, it is missing a piece of the cabinet face and a drawer knob. I will be searching to see if I can find these pieces so if anyone sees any, please holler at me. thank you!
