Is this the one? Also called a wrap n' go casserole carrier.
It's close, but not the one I'm looking for, however, I can easily tell people how to modify these directions. I've learned my lesson, be sure to keep the links for free patterns I've used for ages! Lol

Darn! Okay...you said two handles. Can you describe how the handles work? And the brass circles?

I wish I knew a pattern to tell you. Have made this same one from looking at the one my DIL has. Very easy to make. Hope you find the pattern. I found a beautiful glitter cupcake fabric and made the carrier a little larger so DIL could carry large round cupcake container in it for the children's school parties. Wish I had it here to send a pic to you.

Okay, typo by me at first, it does only have one handle. It is simply called the Jiffy Casserole Carrier and when I printed it quite some time ago from online I didn't write the website and I didn't print it on white paper...or I'd just photocopy it. There aren't many steps and the picture of the carrier has fruit on it. I'm soooo frustrated! I wanted to be able to share the pattern and it's hard to do when I can't remember where I printed it from. Why is it that the easiest patterns are the ones we forget where we got them from. Lol. I sure hope I'm not the only one who does this! Thanks for trying to help. When you attach the 3" brass rings, you fold down 2 of the corners and stitch a small triangle to secure each ring.

Here is the closest picture to the one I'm trying to find: http://www.camplebanon.org/files/1048.pdf and then scroll down to Jiffy Casserole Cover. On mine, the trianes holding the brass rings aren't on the outside, they are on the inside but this is pretty much it.

Can you scan it to a pdf file? Then you can share it.
