It is probably because you're using such thick thread. It's hard enough to bury a knot that is tiny. Rob's idea sounds good.

I was thinking that #12 Perle cotton was awfully thick and that might be the problem.

Alex shows how she does a knotless start if you are using a thread thin enough to loop, she also mentions using a sashiko needle. I have tried to include a link to where she describes the knotless start
you have to choose lesson 2 and scroll to about 21: to see how she does it
