here is a link to classic cottons, they have tons of free patterns you can easily alter for a child. just switch up fabrics

I would tend to finds a novelty fabric or panels that reflect interests. I particularly like very large 8 point stars with square panel centers, although I don't see one at the moment. Adam's quilt is one of a pair I designed for my new step grands. My neighbor simply alternates large squares of novelties and plain fabric for her DGSs.

Of course, printing photos, drawings, etc. is also a great way to individualize a quilt.

I totally agree with QM, I love to see quilts with "stars" on them for guys/boys. For some reason they seem very "manly" for me. I would also find a good focus fabric for them. either in their fav color or if they are really really really into a hobby that isn't going to end in around 4 years I would go with that. Sports are all fine and good but if they like outdoorsy stuff I would go with those fabrics. I would also go with marbles or batiks that are not too girly. This is the quilt that I am working on for my son. I don't plan on him growing out of it anytime soon. but then again he isn't even 2 yet. This is for his twin size bed. (attaching in a moment) I took and purposly made it more "big boyish" than kidish because I want it to last for many years for him. well at least until his room color changes.

I did a simple 9 patch using novelty flannel-bugs, frogs and lizards- mixed with coordinating solids. was a big hit!

Most quilts are really designed for women or girls, but it is not too hard to come up with things that are more masculine.
