Our guild , has Spoonflower print ours by the yard , then we cut them to the size we want. Are very pretty and just as we wanted them.

I would be careful about putting labels on charity quilts. As has been said, Quilt of Valor is very particular about that. They have rules that need to be followed. Maybe Linus has, too.
We have been putting just the name of our group on those which have been going to shelters. |

I believe I saw a YouTube about stabilizing fabric on freezer paper and then running it through an inkjet printer. They were ising for printing patterns and it only works for black ink but then you can use the decorative stitches to add flair.
Now for content I would have the group name, a location identifier and then year. If you want to leave a space for something about the pattern that might be interesting. I also put washing instruction s on mine "Machine wash cold, tumble dry, use often" |
Before you finalize the label, make sure the quilt quild agrees with what you came up with.
