I keep my spent needles in an old plastic film container.When it's full I pitch it and don't have to worry about getting stuck. My Rotary blades go back in the plastic container they came in marked with a crayon so I don't confuse them with the new ones.

Yes definitely time to change. If you use pins you have to be careful not to sew over them. The needle hitting a pin will often cause a nick in the needle which in turn could be the cause of the thread pulling on the fabric.
