Need Quilted Name tag ideas?

Has anyone made a quilted name tag for their guild meetings? Our guild has decided we each need to make and wear our own name tag to guild meetings. If we don't we will be fined at each meeting. Luckily I have three weeks until our first meeting where we need to wear our own nametag. Help, can you post pictures of ones you have made? Thank you.

Here is the name tag I made for my guild meetings. Every tag was different. Different sizes, different in every way.

My guild was called the Evening Stars. Each new member would get a little baggie with the pre-cut pieces of an evening star quilt block. You were required to make the block, add batting, make like a mini quilt. We would write our first name in the center with a sharpie permanent marker. If your guild is permitting you to make your own, then any basic block with a light color patch for your name would work.

Make one using one of the many tutorials for Artists Trading Cards. They'd be about the right size for a name tag.

I just made a little rectangle and appliqued a tiny cardinal (love em) and embroidered my name

Here's a simple tag I made at a workshop before I joined the guild:

We used a paper pieced block for name tags.

Our guild members are given a piece of muslin with our guild name and emblem printed on it (Tree City Quilters from Gainsville, Fl) then we get to make our own. I would show mine, but it's not finished.
Btw, I plan to use my dh's magnetic name tag from his old job to afix it to my clothing. I made a block and then made a sort of slotted back with muslin (like you would a pillow sham)and sewed them together, turned it, and then I am going to slip his name tag in there and put the magnet on the underside of my shirt.
