I did a search for your post, but came up empty. Here is a link with a picture and pattern that may work for you. I have no idea what they mean when they say, 'quilted in the hoop'.... Good luck on your search.

It means that it is a embroidery design that is embroidered in the embroidery hoop on the embroidery machine.

Okay, now I am confused. Is it a design that is embroidered on a single layer of fabric or through 3 layers (fabric, batting, fabric)?

It is stabilizer, backing, and then the fabric with the design embroidered on it.
