Most definitely include picture, name, date, etc. I have a picture of my deceased fur baby framed. I had a small gold plate engraved with her name, date of birth and date of death. It is a sweet memorial.

I vote NO....it will just be a painful reminder every time she sees it....really....I lost my pal in Feb and every day I look at her pix I cry all over again...she has her memories, she does not need a pix to remind her of that fateful day!

Being a dog lover and losing pets I would love to have the picture on the quilt. It will give her some comfort

I too, am one who thinks a picture on the back separate from the label would be good. Then if it was too painful, she could remove it. Although I doubt she will. Time passes and wounds heal and I think whe will love this quilt and you as a wonderful reminder of a dear pet. I think most of us realize that our pets do not live as long as we do. We are just glad to share their lives for as long as we have them. You are a very kind person for doing this and the quilt is so lovely!

You could also add the poem about the Rainbow Bridge. I know it's a comfort to me when I've lost pets.

The Rainbow Bridge
inspired by a Norse legend

By the edge of a woods, at the foot of a hill,
Is a lush, green meadow where time stands still.

She might still love her old quilt, too - so be prepared for her to continue to use it at times.

Sort of like the old, broken in jeans as compared to the new ones.

What a wonderful gift!
One of my favorite pictures of Tasha, a Shiba I had prior to my current two, is with her best friend, a Cocker named Ginger. The two of them sitting together is a happy memory of how much they loved being together to enjoy each other's company. Perhaps a photo of her two dogs together would be also, knowing how much they meant to each other too. Someday, both will be gone and then the photo becomes a bittersweet memory of a time in her life and her two best furry friends.
The date you made the quilt (i.e.: 2011) would be all I'd recommend. Plus a title for the quilt, such as one of the poems/sayings mentioned in other responses?
