I tried posting a site showing a picture of a quilt. I assume it is a quilt. For some reason it didn't take. Sp
search Modern Quilt guild and click under Connecticut. Their logo appears to be one of these quilts.
I see above that my message took. So ignore this one.

I had to go look it up too. I have to admit that I didn't have a clue as to what you were talking about. Looks interesting. Would like to see one made up. If you make one, be sure to post it!!

I just heard at my LQS this evening that there is a free website that will turn it into a grid for you but no one knew the info. :-( if I hear any further I will post, because I really want to make one.

The cool thing about a QR Code quilt is you can put anything (I think there is a limit of letters, maybe around 250-ish) into code so you can make one that says Happy Birthday Joe, or a marriage date with names, or congrats on retirement, or a name, or just nonsense. Anyone with a smart phone can download a program that "reads" the code and the saying will appear on the phone. My granddaughter is graduating from high school this year and my nephew from college and I am considering making one for each of them with the name of their school and graduation date. I just have to get my DH to help me with the math and graphing and them pick the colors.

I generated a QR code and than made a grid. All squares are the same size so the grid is simple. Count the number of squares in one direction and multiply by inches finished. If there are for example 30 squares x 2" = 60" width. With seam allowance of 1/4" your squares need to be 2.5 x2.5". This is just an example. I do not have my design book in front of me. Good luck! I am in planning stages as well. I have no time right now for such an involved project.
