I belong to the Am. Sewing Guild and they provide the labels for you to put on charity projects they are organizing. If I'm doing projects for another organization, I put a label on acknowledging that organization. If I'm just randomly giving stuff to charities, I don't put a label on it.

I just put two iitials of my name. otherwise don't identify them. i believe it is a security issue.

every quilt i make has a label that contains at the bare minimum- my name, city, state, date and CARE INSTRUCTIONS/FIBER CONTENT.
i list if i used cotton or poly batting, cottons or blends, and how to wash/dry it
if the quilt is for a specific person or event i may add that to the label but the minimum is always there
