A Gizmo cutter. Thread - I love Aurifil which is expensive so I ask for it from my DH. A bag of clean mascara brushes to clean lint from her machine. EQ8. Misting bottle.

Spools of different brands of thread (so she can choose which she likes)
Rotary cutter and refill blades
Gift cards to her LQS, an online store, or JAF/Hobby Lobby etc.
Fat quarters
Patterns
Magazines
Baby pony tail elastics (fuzzy ones) to keep around bobbins
Bobbins for her machine
Decent scissors
2.5 x 2.5 inch ruler (or any other size)
Quilting Journal (a nice notebook for recording her quilts)
Pigma pens
Straight pins (1.75 inch quilting pins)
Jelly rolls
Wallpaper seam roller (helps to flatten quilt seams)
Best Press
Jim Shore figure
Quilt calendar
Bingo wand or extendable magnet (I got mine at Ocean State Job Lot - $4 and has an LED light, too)
Pounce or marking pencils & sharpener
"Klutz" glove (Fons & Porter)
Flat head quilt pins (maybe some with numbers?)
Teflon pressing sheet
Needle threader
Small soft paintbrushes to clean her machine

She is a brand new quilter, just working on her 2nd quilt. I'm sure she will be thrilled with anything he chooses for her.

0.4 mm (not the 0.5 mm at JoAnn's) Clover blue and yellow glass headed pins for piecing.

I have a metal yard stick that I find to be more accurate when I'm measuring a flimsy, getting ready to add borders. Maybe it's because it is thinner than a wooden one?? And a quilter can use several of those magnetic pin holders!

Batting, Cutting mat that rotates, Good Iron w/Multi Steam Holes, Small Steamfast Iron to keep next to you w/small ironing mat (June Taylor), Fons/Porter Design board or make her one, foot petal cushion to stop traveling, patterns, Gift Cert to Craftsy so she can buy classes or Iquilt classes, Jelly rolls etc..., sewing machine travel case, ruler organizer holder, tool organizer, bobbin holders, wonder clips, Roxanne Temp glue, Starch, Iris Boxes from Joann's or Michaels to hold stuff i.e. fabric. pin cushions, side thread spool holder, side bobbin winder, side lighting, electric seam ripper (love mine), books...could go on but I think it's a good start

colored pencils (the better ones - such as Prismacolor)
General white chalk pencil or General white charcoalpencil
General Sketch and Wash pencils

a beginner how-to book - Quilts! Quilts! Quilts! is still one of my favorites - I think it's still available through Amazon.

Most beginner books have a listing of equipment/supplies that are needed/useful.
