Please everyone look at the new form. I know everyone probably has all there info saved on their machine cause I do too but the form has changed. Also need your opinion on it.

Verna - I like the new form. There are two questions that I think could be combined and I mentioned them in my PM to you. However, after I filled out the form it still came in too large to send. So I converted it to plain text and it went through.
