I will happily share the name of the pattern with you. It is called Twister and it is put out by Pieced Tree Patterns. It was on a small laminated card.

It was very easy!

The site will direct you to www.thequiltedforest.com to order the patterns. They are listed under "Tiny Ones" and there are quite a few to choose from. I just ordered Twisters and one other one...thanks!!!!
