I had that problem too. I noticed my thread was rising off the spindle. I started putting a cap in the upright spool and now it winds evenly again.

try bluing powder and then dry in the sun. There are some other commercial stain removers but depending on how set the stain is, it may just come back in a few months.

I pieced mine with both extra pieces of thirt and with cotton. I stabilized the cotton with the same fusible I used on the shirts to keep everything the same feel and thickness.
