I think it might be an original design. I would use a regular pieced maple leaf block in the center or applique one on the white, do the Log Cabin and plain blocks to assemble it and then do poppy appliques.

It runs in my mind that Craftsy has a free poppy pattern. I can't check right now because I'm at work and it's blocked, but I think they may have one.

Editted to say: Got in through a back door...There is a paper pieced Poppy pattern and it is free. And, there are maple leaf patterns, too.

I agree ... it is most likely a one only!
Perhaps your cousin could contact the Legion that showcased it and ask for further details?
Some Legions have even made patterns for sale as fundraisers.
Here's a PP maple leaf that I have used several times.
I adjust the size according to whatever works for the project at hand.

Here it is in my version of the FWS Pony Club quilt ...

I also believe it is an original design. It would be fairly easy to somewhat copy. For the center block, I know there are plenty of free maple leaf block designs out there. The surrounding blocks are log cabins with what looks like an applique of a poppy leaf over it, aka broderie perse. The hardest part might be finding a fabric print with large poppies.

From the bottom of the picture, it looks like it has something to do with the Royal Canadian Legion. Here is contact info for them: http://www.legion.ca/who-we-are/contact-us/ They might be able to give contact information for the quilter.
You might ask him what magazine it came out of and contact them for information or contact number for the quilter? There is also a quilting magazine for Canadian quilters http://quiltersconnection.ca/shop1/ They might be able to give you some information. Hope you find out. It is a nice quilt.

I'm wondering if the "maple leaf" is a special design with significance? When you look at it closely, there is a pine tree, the top resembles a crown and the side portions suggest angel wings.

Good for you for catching that. QuiltE's maple leaf has almost the same design.
