I've not bought any from them, but have received some of their fabrics in I spy swaps. It has held up well.

I have not. I pretty much stay with 2 sites. http://www.fabric.com and http://www.5bucksayard.com.

Yes. I have created my own designs to print and purchased yardage of my designs, and I've also purchased fabric by other designers. I've been very happy with both.

Yes, very nice fabrics at a very high price. I ordered the Kona Cotton version of multiple prints that have held up well so far (several washings). The few times I’ve ordered was due to specific print requests only found at Spoonflower.

Price is prohibitive for me but sometimes you have to pay to get exactly what you want. Have had friends who have used the design services and who were very happy. Don't have many reports to go on (3-4) but everything went well and exactly as expected.

I was very happy for the fat quarter I used to make the tiny log cabin centers for the quilt show awards.

I bought from them once when I needed a very specific fabric for a project. Very expensive.

I have and I recommend the Kona cotton for quilt projects. The cheaper cotton was not as nice. I didn't think the Kona was super nice either, but it had the design I wanted and it has held up. I usually only buy when they have the BOGO fat quarter sales. I've also bought their jersey knit and I loved that! Nice quality, but as others have said it was pricey.

Also, they recommend you wash fabric before you use it. I suggest finishing the edges before you wash.
