I usually will embroider this information on the backing material before applying it to the quilt.
I have also put the information on a piece of material and hand sewn it on the back once the quilt is finished.

I have created labels in several ways. You can embroider a label, or you can write with a Sharpie or other permanent, non-bleeding pen (I set the ink with an iron). You can also print a label on fabric using Bubble Jet set and your computer (directions are on the bottle). I think labels are very important. Be sure to include, at a minimum, who created and/or quilted the piece, the city/state and the date. You can also add the recipient's name, the event, a quote, or a reason for the quilt. Be as creative as you want!

There are many ways to make labels ---- I use a solid color fabric (usually white) - cut to the size I need - spray starch - turn under the edges all around for neatness sake - draw a border design like entwined hearts, vines and flowers, etc... in pencil -- add my name, date, quilt name, and any other info in pencil - then I trace all the pencil lines with colored fabric markers - heat set with iron - hand sew to the back of the quilt - done!
