I use the surgeon's knot too and I usually put my ties on the back.

We tied a couple of quilts with Pearl Cotton, and many of the knots came loose. We retied them and put fray check on each one. What a pain. I have always used yarn; any kind. If you use wool yarn, they get tighter with each wash, and never come loose.

I've done them with DMC embroidery thread, using all 6 strands, with the ties on top, or like above posts mentioned, machine tack in random places enough to hold it together for washing.

Instead of tying my baby 'blankets' (usually two flannels), I use my embroider machine and embroider an almost 4" chicken scratch heart in various places on the blanket to hold the flannels together. Then I put blanket binding around it. I remember MY babies liked that satin binding.
