While holding both top and bottom threads, I hand turn my needle two or three times in the same hole. That should lock in the beginning stitch........when I stop, I do the same thing in the last stitch, hand turn the needle a couple times in the same hole.....then clip......

    Thats how I do it and I haven't had problems with stitches coming out.
