I too have had this problem. Sometimes it just does not like the thread you are using but more times it is the needle. Mine likes a 90 U for most quilting with my BSR. You just have to keep trying...

Stupid Me! Got brown stuff from iron onto appliqué

So mad at my stupidly tonight!! I was so careful using non stick appliqué sheet to keep fusible gunk off my iron. Don't know how or when but got some on iron then pressed some brown gunk on the front...
