You'd need the belt guard / bobbin winder with screws, hand wheel with stop motion knob and washer, light assembly (might could still use your original one) and the appropriate screw. Also the motor with mount and screw if you want to motorize it. (I stole your pic of the front of your machine to illustrate the screw holes. I didn't have one of my own.)
It's actually very easy to do. The best way would be to buy an inexpensive clone or other similar vintage Singer, and higrade the parts.

nope, what joe is say you need, ia belt guard / bobbin winder and a spoked hand wheel. he's leaving out is the thumb screw and washer, motor , belt bolt and wiring. annnd in your case the hand wheel adapter.
meaning every part from the machine body out
after your posy yesterday I look again in baton rouge C/L , with in guessing 60 miles of you there are 3 machines all under $40, that will give you all those parts. One machine is $20 and very nice looking machine.
These are all japanese clone 15"s which are the same as a singer.

The only hard part is to get the belt guard screws. If it has a bent shaft the hand wheel goes on you are out of luck - see if it turns with out the hand wheel.
Also - some of the clones do not have belt guard screws - if the belt guard is one piece on the top of the machine you can not expect to find screws to hold on a belt guard for a Singer 15.

10x,
I think you should reread my post. I did include the motor. Many of the other parts are already there. Why would you need thumb screws that are already there?
Miriam brings up a good point. Considering the damage to the hand wheel and motor, the shaft could be bent. At that point it's basically a parts machine.

As for the mounting screws, that's why I said to find a donor machine if possible.

joe your post came in between the time I started my reply and the time I sent it.
often on this site people call the stop motion knob and thumb screw and a few other terms. as in big round thingy.
as I ask in the start of this thread . can you turn it over easily ? if not teh upper shaft is bent , she doesn't answer that.
none of the machines I just posted are parts machines, examples of what you can get and at what price in her local area. should a person chose to repair that machine ( 15-91), using one of those is a much more logical way to source parts , opposed to using ebay or ? and paying 200 x's the price of those machines.
