There are a lot of strong feelings here, and I can see why. I do think any time one of us delivered a quilt to be quilted we would "think" it was ready. But then we all know how easy it is to not see that mistake that becomes glaring in another's view. I find viewing photos of my quilts throughout the process helpful in keeping things as they should be.
I can understand not seeing a mistake and quilting over it, however, I have to agree with those who have said if a quilter saw my mistake and didn't notify me so I could fix it if I chose to, I would be very upset. These 2 blocks are glaringly wrong and were the first thing I saw as they throw the symmetry of the top all off. I would have a hard time imagining that it was done intentionally and would have had to ask first.
No denying the beauty of the quilting however.

Oh my! If I load a quilt on my rack and I see something like that, I assume nothing! I would have contacted the client immediately to give her or him the option of fixing it, paying me to fix it or leave it as is. Your quilting really compliments this top but if this was mine, I would be heartsick to see my mistake after it is already quilted and much harder to fix.
