I believe the name of this bag is "Perfect Patches", I have made it and the pattern was in an Australian Patchwork & stitching magazine quite some time ago.
Just looked at the site for the Sweet Sixteen Bag and compared this pattern to mine. The Bag I made that looks very similar to this one is called Perfect Patches and is made from 22 x 5 inch squares.
