I think, almost by definition, that a list of this sort will always be incomplete.
Yes, it will always be incomplete, but it will be the most comprehensive available if there is good participation and it will have dates of inclusion.
I don't understand your confusion between 3 and 4. Three is judgemental and four is any specialty offered.
