I usually just make mine from a prewashed fabric that I like. And I don't use a pattern I just lay the fabric out on the ironing board and cut about 4" below the top. Then I hem it with a cord inside the hem, put it on the board, pull the cord and tie it. You do have to put a little reinforcement right where the cord comes out to keep it from tearing.
