For the blankets I use an awl to punch the holes. I tried the rotary cutter but didn't like it. I punch the holes about 3/8 of an inch from the edge and about 3/8 of an inch apart. I do different patterns depending on my mood or the design in the fleece. I use yarn, mostly worsted weight though I have used baby yarn with a smaller hook on some. I like worsted weight better. They go faster. And since I make these to sell my time is valuable. I have probably made 300 blankets in the past 3 years. That is alot of holes punched.
By the way, I use a yard of fleece and cut in half length-wise so I get 2 blankets per yard.
