I found the pattern for the checkbook online here:

http://amatricis.wordpress.com/2008/...book-tutorial/

The key fob I just made up myself.
I fashioned it after a luggage tag I have...only not as large.
(I give terrible directions but here goes)

Cut a circle out of cardboard the size you wanted the fob, using that as a template, cut 2 circles of stiff fusible web the same size as the cardboard.
Press onto fabric(s) for front & back of fob. Cut the fabric 1/2" LARGER than the fusible web.
Take a running stitch aound the edge, then place the cardboard inside (temporarily)
Pull thread and gather the edges. (similar to making a yo-yo).
Press edges down, remove cardboard. Make sure gather is tight and tie thread ends. *

Cut a 1" x 2" strip of fabric, place wrong sides together and sew down the length. Open so seam is in the center and press.
Then fold in half lenghthwise (seam to inside) and sew along the long edge. This will be the tab that attaches the ring.
Put metal ring on & fold tab in half.
Place the two circle pieces WRONG side together, insert tab with ring where you want it positioned,
then sew around the edges attaching the two circles together.

*(If you want to add rick rack or lace I usually baste-stitch it to one of the circles at this point.)
