Lots of people just write the details on a piece of fabric. You need to use a marker that is fabric friendly, so it doesn't run or fade.

I sew that to another piece of fabric (with the information on the inside) and leave an opening so that you can turn it right side out (then whip stitch it shut).

Then you can hand tack that to your quilt.

I recently learned how to put the information in the computer and print it out on fabric that can be run through a printer. Got the "Sew-in Colorfast Fabric Sheets for Ink Jet Printers" by June Tailor, Inc. at JoAnn's.
