When I tie quilts, I knot them tight and trim the ends to about 3/4 inch. If you are concerned with getting thread wrapped around little fingers or toes, cut the thread ends to 1/4 inch.
That was my experience too. a thread wrapped around the end of a little toe inside a sleeper. I've never had an issue with tied quilts or rag ones. Been making them for years.
