So, let me get this right:

1. Row of blocks plus whatever sashing

2. Small journal to be filled in as the next row is worked on, pictures welcome (could probably be printed on regular paper direct from computer, couldn't they? Not necessary to buy prints.)

3. Quilt-owner's big label -- description of project with four lines for signature only -- real and board names, location.

4. And then I'm a little fuzzy about the 4th item --

Couldn't we combine 3 & 4 -- each person can make a 12 1/2 inch block, maybe out of muslin trimmed or framed in with some of the fabric they used on your blocks (or a small block motive on edges) and leave a big space in middle for their names, dates, home town and state, and any other very short comment they want to make. A plain muslin 12 1/2" block can be made by the quilt owner stating the name of the swap and date range -- combined with the other four blocks would make a complete row across the back. Do I make any sense at all?
