I find that most puckers are caused by different weights fabric. Example: Sew a strip of all cotton fabric with a strip of cotton gingham, you will have puckers.
Just try to stay with the same weight fabric and if possible use the same manufacturer for the whole quilt.
This is difficult to accomplish, but worth the time and effort.
