I agree that it is probably a paper pieced pattern. Each of the four points of the star would be a square and then 4 would get sewn together.
Actually, i think the star is a secondary pattern. I'm pretty sure that the blocks are the 'ray' blocks, and how they come together forms the star at the intersections. And I'd paper-piece it, too, so I know all my seams line up!
