You can download the forms and get information at
http://travel.state.gov/passport/get.../first_830.htm , if you have never applied for one before you do have to submit it in person along with certified copy of Birth Certificate and picture ID, they don't accept Credit Cards. Just an FYI. Good luck. and they aren't cheap!!!!
