Generally speaking, if the bottom of the piece is messed up, the fault is with the top tension. If the top of the piece has the mess, the bobbin is at fault. It sounds like your presser foot is not down or the top thread is not correctly seated in the tension disk. Is there a broken thread in the tension disk? Loosen it and brush it out well.
