I haven't, but I've only made one quilt that I actually put a label on. I should put a label on the other two quilts I made in past years though.
I'm not sure what name to put, I probably should put all my names on it - maiden, ex (my children's last name), and current, but that would be rather cumbersome. However, from a genealogy point of view, it would make sense. (And my mother was a genealogist, so she would appreciate it. In fact, in her later years, she went by her maiden name followed by our last name - "First name, MI, Maiden name-married name. That's what's on her tombstone at her request.)
Someone mentioned that in other cultures, women do not change their names. I worked with Burundi refugees, and this is their practice. To many of them, their name is part of their identity, and changing it to their husband's name is unthinkable. The children in these cultures take the father's name.
