I definitely agree with getting a new culture, even if you've done it once already. I have beagles and one of mine almost always had a yeast infection and another ALWAYS had bacterial infections. The treatments are completely different.
Daily cleaning for a few weeks will help once the right medication is available; your vet should have a cleaning solution you can try.
