I have a friend who is a longarm quilter. She loaded a customer quilt on her frame and was humming and quilting along. Unfortunately, a miller moth lit on the back on the quilt as she was stitching and she quilted it right through it into the back of the quilt. She of course didn't see it until she advanced the quilt. It took a little picking out and cleaning up, but everything turned out OK (probably not the moth's opinion).
