The volunteer office at my local hospital has labels to put on the quilts. They require this rather than a personal label. I do not put my name and other personal information on the label. Why don't you call the hospital and ask if they have requirements.
I always put my initials with permanent ink in a corner on the back or stitch the initials if dark fabric.
