It is sitting on an oak treadle machine which may have confused you...sorry. I'm sure the case is complete but cannot explain the holes. I think you are probably right about the smaller belt so will try to figure how to get one on. When I bought it, my concern was the motor which works fine when the dealer plugged it in.
