if you take a look at page one of this swap there is a template that you will need to complete with your information and then send this template by private message (pm) to the swap host lori peercy. what many of us have found helpful is if you highlight and copy the template to a word program, type in all of your responses, then save the template in your word program. you would then copy that template into a pm to lori.

please do not post the template with your personal information to this thread for any reason. i do not know how doing this on your phone will work. it may be a bit difficult.
