I have a Gammill and sometimes find this mess in the last twelve or so inches I've quilted. The problem? My thread comes out of the tension easily but will carry on like it's fine. So you have to be vigilant...and I also laid some mirrors on my table to watch the back more closely.
I use a hand mirror too, with a small light attached to I can check the bottom tension. Much easier that trying to crawl under the quilt, turn your head at a funny angle to see the bottom stitching.
